How to Book for PFIZER Vaccination

1. Please DO NOT CALL our medical centre.
2. Book your appointment online. Visit our website: www.allcarecarneshillmedicalcentre.com
3. Check your eligibility on the Eligibility Checker.
4. If you are eligible, proceed as follows:
5. Please Click on the Make an Appointment Tab.
6. If you are an existing patient, Select: type of appointment as: PFIZER DOSE1
7. Complete the Consent Form and rest of the steps as prompted.
8. Select the date and time slot available. Your Booking is done. You will receive a confirmation SMS.
9. If You are a NEW Patient: Fill up your details in New Patient Form. Complete the Consent Form.
10. Select the date and time slot for Pfizer vaccination. You will receive a confirmation SMS for Pfizer Vaccination Booking.
